Salary Range: $125,000 - $135,000 per year
Bonus / Commission (if applicable):This role is eligible for a sign-on bonus or loan assistance of $15,000, paid over 3 years.
Location: Bronx, NYField-Based / Onsite
Position Summary:
As a Nurse Team Leader in Home Care, you will provide clinical oversight and leadership to a team of field nurses while ensuring high-quality, patient-centered care for individuals in the community. This role focuses on managing care plans, mentoring staff, and coordinating services with interdisciplinary teams and families. You will play a key role in maintaining clinical excellence and supporting the delivery of home health services.

Key Duties:
- Oversee and manage individualized patient care plans to ensure quality and continuity of care
- Supervise, mentor, and support home care nurses, including onboarding new team members
- Coordinate with interdisciplinary teams, patients, and families for comprehensive care delivery
- Document care activities and maintain accurate records using EMR systems
- Serve as a clinical leader in the field, providing guidance and ensuring compliance with home care standards
Requirements? Education
Required: Associate's Degree in Nursing
Preferred (if applicable): Bachelor's Degree in Nursing (BSN)?
Licenses & Certifications
Required: Active New York State Registered Nurse (RN) license?
Skills:
- Strong clinical expertise in home health and regulatory standards
- Leadership and supervisory skills with ability to mentor staff
- Proficiency in EMR systems and Microsoft Office
- Excellent problem-solving, communication, and organizational skills
