Home Health Registered Nurse

Provide home-based nursing care by assessing patients, coordinating and implementing care plans, teaching self-care, and communicating progress with the care team. Supervise LPNs and home health aides, complete clinical documentation, and provide on-call support as assigned. Assess patients and collaborate with the care team to develop and implement care plans. Refer patients to other disciplines based on identified needs and document referral decisions. Teach patients self-care techniques and promote health, independence, and rehabilitation. Supervise LPNs and home health aides. Complete timely, accurate documentation and communicate patient progress to the clinical manager and care team. Provide staff education and fulfill assigned on-call responsibilities.
