Key Responsibilities:
Conduct thorough annual home health risk assessments for Medicare Advantage plan members in Rock Island County, including the communities of Rock Island, Moline, and East Moline.
Review and evaluate medical histories, ongoing conditions, and current medications.
Educate patients about their health conditions and prescribed medications.
Identify gaps in care and provide informed recommendations for improvement.
Perform post-discharge visits as needed; evaluations typically last about 45 minutes.
Focus on critical assessment and patient educationno prescribing or creation of treatment plans required.
Qualifications:
- Unrestricted Nurse Practitioner License in Illinois
- Master of Science in Nursing (MSN) from an accredited program
- Board Certification through AANP or ANCCCurrent BLS (CPR) certification
- At least 6 months of recent NP experience preferred
