In-Home NP/PA Clinician: Compassionate Health Evaluations

Signify Health is seeking a Full Time Clinician (Nurse Practitioner or Physician Assistant) to provide in-home health evaluations for members in their own homes. The clinician will build trusted relationships and help bridge gaps in care, visiting locally and, as needed, in licensed states. Role requires 40 hours per week, board-certified NP/PA with an active license. Travel and virtual visits may be required; comprehensive benefits accompany the position.
