As our new Physical Therapist (PT), you will play a vital role in helping our residents achieve their rehabilitation goals and regain independence.
About the Role: As a Physical Therapist, you will be responsible for:
- Conducting thorough evaluations and assessments of residents to identify and develop personalized therapy plans
- Providing individualized physical therapy treatments to help residents achieve their goals, including strengthening, flexibility, and mobility exercises
- Collaborating with other healthcare professionals to develop and implement effective care plans
- Monitoring and documenting resident progress, adjusting treatment plans as needed
- Educating residents, families, and caregivers on proper techniques and exercises for continued progress
Key Responsibilities:
- Develop and implement individualized physical therapy treatment plans for residents
- Conduct thorough evaluations and assessments to identify resident needs and develop goals
- Provide individual and group physical therapy treatments, including exercises, stretches, and other interventions
- Collaborate with other healthcare professionals to ensure comprehensive care
- Document resident progress and adjust treatment plans as needed
- Communicate effectively with residents, families, and caregivers to ensure successful outcomes
