Summary: Lead a stable and well-established residential program supporting two individuals who are active in their community and also enjoy time at home. Both individuals have strong family connections and regularly spend weekends with their families. One resident is highly independent in many areas and naturally supportive of their housemate, while the other is experiencing increasing medical needs. The Program Manager will help preserve the program’s steady, supportive environment while coordinating care, maintaining strong communication with families and the residential team, and ensuring services continue to adapt as individual needs change. This opportunity is a great fit for a leader who values relationship-building, consistency, person-centered support, and proactive care coordination.
