Signs of Life 2, LLC is seeking a compassionate Registered Nurse (RN) to provide community-based nursing services to adults enrolled in the Home and Community-Based Services–Adult Mental Health (HCBS-AMH) program in Houston and Harris County. Use your nursing skills to help participants understand their health needs, manage their wellness, and work toward greater independence.
Position Details
Pay: $43 per hour
Subcontractor opportunity In-person services in Houston and Harris County
Responsibilities:
Complete nursing assessments and monitor participants' health needs.
Provide education about medications, health conditions, and wellness.
Identify and communicate changes in participants' health.
Coordinate with service providers and the participant's recovery team.
Complete accurate, timely nursing documentation.
Provide services according to each participant's recovery plan and applicable nursing standards.
Qualifications:
Current RN license authorizing practice in Texas.
Current CPR/BLS certification.
Strong communication, organization, and documentation skills.
Reliable transportation to participant visits.
Mental health or community nursing experience preferred.
Ability to complete required background screening, training, and onboarding requirements.
