Family Nurse Practitioner

Elevance Health seeks a Family Nurse Practitioner to deliver high-quality, whole-person primary care on a flexible PRN basis. In this role, you’ll provide assessment, diagnosis, and treatment for diverse populations across the lifespan, focusing on prevention, chronic disease management, and health education. Collaborating with interdisciplinary teams, you’ll use evidence-based guidelines, EHR systems, and telehealth tools to coordinate care, close gaps, and improve outcomes. Provide comprehensive primary and preventive care across the lifespan, including assessment, diagnosis, and treatment. Manage acute and chronic conditions using evidence-based guidelines and value-based care principles. Document visits and care plans accurately in the electronic health record (EHR). Collaborate with physicians, nurses, behavioral health, and care management teams to coordinate services. Deliver patient education and coaching to support self-management and whole-person wellness. Utilize telehealth and digital tools to expand access and close gaps in care. Order and interpret diagnostic tests and adjust treatment plans accordingly. Ensure compliance with clinical, regulatory, and quality standards. Participate in population health initiatives to improve outcomes for diverse member populations. Flexibly provide PRN coverage to support clinic and organizational needs.
