Home Health Physical Therapist Assistant

Addus Family Company is seeking a compassionate Physical Therapist Assistant to support home and community-based rehabilitation for seniors and individuals with disabilities. In this role, you will implement treatment plans under a Physical Therapist’s direction, focusing on safety, mobility, and independence in the home. You’ll provide hands-on therapeutic exercises, gait and balance training, and patient/family education while accurately documenting progress. Implement treatment plans under supervision of a licensed Physical Therapist Provide therapeutic exercises, gait, balance, and transfer training in home or community settings Observe, measure, and document patient progress and response to treatment Educate patients and family/caregivers on home exercise programs and safety strategies Communicate patient status and concerns to the supervising Physical Therapist and care team Assist with fall prevention, mobility, and functional independence goals Ensure compliance with clinical, regulatory, and documentation standards Maintain a safe, respectful, and client-centered care environment Participate in ongoing training and quality improvement activities Coordinate scheduling and visit frequency in collaboration with the care team Therapeutic exercise implementation Gait and balance training Functional mobility and transfer training Home safety assessment basics Patient and caregiver education Clinical documentation and EMR use Vital sign monitoring and basic medical observation Knowledge of rehabilitation and therapy equipment Care planning under PT supervision Time management and route planning for home visits Medical Insurance Dental Insurance Vision Insurance Life Insurance 401k Current state-licensed Physical Therapist Assistant (PTA) Graduation from an accredited PTA program Current CPR/BLS certification Valid driver’s license and reliable transportation Ability to pass background check and health screenings
