Shift Description:
Monday-Friday 8a-4p; no weekends, no call.
OH license - Required, not accepting IMLC.
Board Certified - Required.
Clean background - Required, no exceptions.
Covid Vaccine - Required at time of name clear.
Availability: must be full time M-F 8a-4p - Required, no exceptions.
Schedule: Monday-Friday 8a-4p; no weekends, no call.
Practice setting:
Office/outpatient clinic.
Percentage of adult patients: 100%.
Percentage of outpatient patients: 100%.
Average number of patients/day: 14.
Other Neurologists will be present during Locum's assignment.
12 Physician's and 4 APP's in group.
Office staff: 2 MA, 2 front desk.
Documentation system: EPIC.
Trauma Level/designation: not designated.
Credentialing timeframe minimum 120 days - MUST have paperwork in to expedite, can request temps with a clean file.
