What You'll Do and Our WhyWhat You'll DoAs a Transitions Hospice Night Nurse, you'll serve as a critical support member of the interdisciplinary care team during overnight hours — assessing, treating, and communicating patient needs with clinical confidence and genuine compassion. Your responsibilities include: Delivering professional nursing care across the full nursing process within your scope of practice Conducting thorough patient and family assessments and contributing to individualized care plans Completing all visit types as needed to meet overnight patient and family needs Administering medications and treatments per physician orders Identifying nursing diagnoses from health assessment data and responding with sound clinical judgment Recording accurate, timely clinical notes including pain and symptom management outcomes Communicating changes in patient condition to physicians and obtaining orders as required Coordinating with community health partners to support continuity of care Ensuring equipment and necessary services are in place for patients Maintaining and growing your clinical skills through continuing education and in-service participation Why Choose Transitions Hospice? Skills, Knowledge and Expertise What You Bring Active RN license in the state of employment Graduate of an accredited school of Nursing.
