APPLY NOWOutpatient Stroke APP – Scottsdale, Arizona HonorHealth Medical Group is seeking a full-time Nurse Practitioner or Physician Assistant to join its growing outpatient stroke program at the HonorHealth Neuroscience Institute at Osborn. The APP will work alongside two experienced stroke neurologists and primarily care for patients requiring ongoing management after stroke, hospitalization or other cerebrovascular conditions. Responsibilities:
- Evaluate and manage patients following stroke or hospitalization
- Provide timely post-discharge follow-up care
- Care for patients with stroke, TIA, carotid artery disease, cerebral blockages and a history of hemorrhage
- Review imaging, laboratory results and hospital records
- Develop treatment and secondary stroke-prevention plans
- Educate patients and families and coordinate care with the stroke team
- QualificationsCertified Nurse Practitioner or Physician Assistant
- Current or eligible Arizona license
- Neurology, stroke or related experience preferred
- Strong clinical judgment and a collaborative approach to care
