Job Description:
Deliver quality care and make a difference in patients lives as an experienced Registered Nurse providing home health services. This critical field-based contract position involves performing skilled nursing duties, individualized assessments, and collaborating closely with interdisciplinary teams to deliver comprehensive, patient-centered care in the home environment.
Schedule: Monday to Friday, 8:00 AM 5:00 PM
Key Responsibilities:
Conduct comprehensive OASIS assessments and develop individualized patient care plans.
Deliver skilled nursing care, including medication administration, wound care, IV therapy, and chronic disease management.
Monitor patient progress, identify changes in condition, and communicate promptly with physicians and the care team.
Educate patients and families on disease processes, care plans, medications, and preventative measures.
Accurately document all care and interventions in the Home Care Home Base (HCHB) electronic system.
Collaborate efficiently with therapists, social workers, and home health aides to ensure coordinated and effective care.
Participate actively in case conferences, team meetings, and ongoing education initiatives.
Adhere to all applicable clinical and regulatory standards for home health care.
Qualifications:
- Active RN license in the state of practice in good standing.
- At least 1 year of recent field-based home health nursing experience.
- Proficient in OASIS assessments.
- Previous experience with Home Care Home Base (HCHB) documentation.
- Strong clinical assessment, critical thinking, and communication skills.
- Self-motivated, organized, and able to work independently.
- Reliable transportation, valid drivers license, and auto insurance required.
- BSN and prior experience managing 2532 points per week are preferred.
- Familiarity with Medicare and home health compliance standards is a plus.
