UCHI United Church Homes, Inc. – Trinity Community at Fairborn, Ohio – is seeking a Staff Nurse (LPN) to coordinate care for about 25 residents within approved nursing protocols and care plans.
You will assess residents, administer medications, monitor vital signs, assist with treatments, document progress, and collaborate with physicians, RN staff, dining services and families to ensure high quality, person-centered care.
