In this role, you will manage specialty patient caseloads, perform comprehensive assessments, develop and coordinate care plans, and provide in-depth education to patients and families. You'll collaborate closely with physicians and interdisciplinary teams, ensure safe transitions of care, and support evidence-based practices. Coordinate care for a panel of specialty patients across the continuum of care. Perform comprehensive nursing assessments and develop individualized care plans. Educate patients and families on diagnoses, treatments, and self-management. Collaborate with physicians and interdisciplinary teams to optimize outcomes. Monitor patient progress, adjust care plans, and facilitate follow-up care.
