The Role:

- What You'll DoRestore Independence: Conduct initial evaluations and developfunctional, goal-oriented care plans.
- Clinical Excellence: Provide one-on-one therapeutic interventions, including gait training, strengthening, and manual therapy.
- Safety First: Perform home safety assessments and recommendmodifications to prevent falls.
- Documentation: Complete timely OASIS assessments and progressnotes via our intuitive EMR system.

Requirements: What You Bring
Credentials:
A degree in Physical Therapy from anaccredited program and a valid state license.
Experience:
Minimum of 1 year of clinical experience(Home Health experience preferred).Reliability: A valid driver's license, auto insurance, anddependable transportation. Tech-Savvy: Comfort with electronic documentation andmobile communication.
