In-Home Health Medical Director | Market Leader

Monogram Health seeks a Market Physician Executive to lead our in-home multispecialty care model in a designated market. You will oversee clinical and operational activities, direct care plans, and collaborate with APPs, RNs, social workers, and pharmacists to improve health outcomes. You will report to the Regional Vice President and shape clinical strategies while managing complexity across multiple sites. Remote work within the market is expected with some travel and on-site visits as needed.
