Home Health Physical Therapist Access TLC Home Health Care is currently seeking professional Physical Therapists in Simi Valley, Thousand Oaks and surrounding areas. Position is per diem/per visit. Visit rates range from $100 - $130 per visit.
Requirements:- Current California PT License
- Valid California Driver's License with proof of a good Motor Vehicle Record
- Current proof of auto insurance with state minimum (or greater) coverage
- Social Security Card and/or proof of eligibility to work in U.S.Two Professional References
- Current CPR Certification
