In-Home NP/PA Clinician (Part-Time, Flexible Travel)

Signify Health, part of CVS Health, seeks a Part-time Clinician (NP or PA) to perform in-home health evaluations for health plan members in Denver/Jefferson County. Visits last about 45 minutes and include a medication and medical history review, a physical evaluation and, when ordered, additional diagnostic tests. You will use an iPad with a simple clinical workflow, and you are not required to prescribe medicines or modify existing treatment.
