Key responsibilities:
Engage proactively with patients to improve healthcare delivery and outcomes
Support the Care Coordination Program across all SLPG Primary Care providers and clinics
Collaborate with stakeholders to maximize performance within value-based contracts
Required qualifications:
Less than 1 year of experience as a Medical Assistant (Certified)
Associate Degree in a relevant field
Understanding of population health management and value-based care principles
