Home Health Registered Nurse (RN)

Hartford Healthcare is seeking a compassionate HomeCare RN to provide skilled nursing services to patients in New Haven and West Haven. In this field-based role, you will conduct assessments, manage medications and treatments, educate patients and families, and coordinate care with a multidisciplinary team. Provide skilled nursing care to home care patients in New Haven and West Haven, following individualized care plans Conduct comprehensive patient assessments, including vital signs, medication review, wound and pain assessments Administer medications, treatments, and therapies safely in accordance with physician orders and protocols Educate patients and families on disease management, medications, and home care best practices Collaborate with physicians, therapists, social workers, and caregivers to coordinate care Document all care, assessments, and patient progress accurately in the electronic health record Monitor changes in patient condition and promptly report concerns or emergent issues Participate in case conferences, quality improvement, and professional development activities Home health nursing Patient assessment Care planning Medication administration Wound care IV therapy Electronic health records (EHR) Patient and family education Care coordination Infection control Medical Insurance Dental Insurance Vision Insurance Life Insurance 401k Active Registered Nurse (RN) license in CT BLS (Basic Life Support) certification Home health or community nursing experience preferred OASIS documentation experience preferred
