RN - Home Health

Description: The position requires current RN licensure in Client. Candidates must have two (2) years of experience in a home setting within the last ten (10) years. Experience can include working with certified home health agencies or licensed home care service agencies. Candidates should have recent experience conducting assessments that resemble UAS, such as OASIS. Responsibilities will include initiating assessments of health status and developing service plans. Nurses will review consumer medical documentation and enter evaluation data electronically. The role emphasizes continuity of care and discussion of healthcare options with consumers. Collaboration with interdisciplinary care teams is essential to improve care coordination. Candidates must identify opportunities for health promotion and maintain knowledge of community resources. Strong organizational and communication skills are required for the role. The ability to work independently in a fast-paced environment is necessary.
