Provide skilled nursing care to patients in their homes under the supervision of a Registered Nurse. Coordinate with the RN and physician, monitor patient condition, teach self-care, manage medications, perform ordered procedures, and document care accurately. Verify the plan of care with the RN before each visit. Coordinate with the physician and RN, reporting changes in patient condition promptly. Teach patients and caregivers self-care techniques. Check vital signs and review medications, including medication reconciliation. Perform physician-ordered procedures and prepare treatment materials using aseptic technique. Complete patient documentation accurately and on time.
