The RN - Case Management - Utlization Review to provide clinically-based case management to support the delivery of effective and efficient patient care.. Registered Nurse with Bachelors of Science in nursing (BSN) degree or Registered Nurse with Associates degree in nursing (ADN) plus five years utilization review or case management experience required.. As a day shift, full-time, Care Manager and Discharge Planner , you would be working for the only Level I Trauma hospital within Albuquerque, NM. Conduct initial and ongoing assessments, initiate disease management protocols, determine and manage outcomes, ensure continuity of care through discharge planning, utilization of resources and analysis of variances.. Patient care assignment may include neonate, pediatric, adolescent, adult and geriatric age groups.. ORDERS AND REFERRALS - Obtain necessary orders from physicians to initiate home health referrals, home infusion medications and supplies, oxygen and equipment; coordinate referrals for oxygen and equipment. DISCHARGE PLANNING - Conduct timely discharge planning by anticipating patient needs in collaboration with physicians, staff RN's, and other health care team members

INTERVENTION - Provides de-escalation, brief interventions, crisis stabilization and case management to community members; Provides in-person interventions, telehealth, phone triage, and support to clients in need of crisis and case management services. TEAM - Works as a team providing mobile Crisis Now response to dispatches coming from 988 and co-response dispatches with local law enforcement coming from 911 as needed to assist in the evaluation and management of clients in crisis in the community; works as a team to also provide internal crisis response at UNM SRMC when requested. INTERVENTION - Provides in-person interventions, telehealth, phone triage, and support to clients in need of behavioral health intervention and follow up. SUPPORT - Provides suicide and risk assessment and abatement, intervention to include Certificate for Evaluation as needed as part of the interdisciplinary team. The Domestic Abuse Victim Advocate (DAVA) provides AF personnel, their family members, or intimate partners who are victims of intimate partner abuse non-clinical emergent and urgent service whenever requested.. Develop process to keep the social work supervisor and designated POC (e.g. on-call mental health provider, emergency room.). At the discretion of the Social Work Supervisor, the DAVA may also provide emotional support and information and referral services to the non-offending caregiver in a child maltreatment or youth sexual assault case.. All DAVA personnel working within an MTF must maintain current certification in either the American Heart Association Basic Life Support Course (BLS) (Course C) or the American Red Cross Cardiopulmonary Resuscitation (CPR)/BLS (Heart Saver) Course.. Before starting work, DAVA personnel working with MTFs shall provide proof of immunization from the following diseases according to CDC guidelines: Hepatitis B, measles, mumps, rubella, varicella, and influenza, and proof of a negative TB skin test (completed within the past 12 months (if positive, proof of negative chest X-ray within the past 12 months) prior to start of work).

The purpose of this position is to provide monitoring and case management services to over 1,000 Protected People that receive professional guardianship services from OOG. This is primarily accomplished through Protected Person Welfare Visits (PPWVs), a periodic face-to-face onsite visit, reviewing and approving vendor's PPWV reports, and following up on any action steps discovered during the PPWV.. Developing the internship program processes and procedures for recruiting and hiring interns; creating and monitoring learning contracts that integrate classroom theoretical knowledge and values with practice skills; training interns on guardianship and alternatives to guardianship; and evaluating the interns¿ performance..
