Teaching (predominantly face-to-face modality) in:Undergraduate Human Services Program, such as required courses in the Human Service Major, as well as the Nonprofit Management and Child Advocacy Studies minorGraduate Social Work Program, such as required courses in the Generalists and Specialist areas of the curriculum.. An earned Terminal/Doctorate degree, or the foreign equivalent, in Social Work, Human Services, or a related discipline (e.g., Education, Child and Youth Care, Counseling Psychology). MSW from a CSWE accredited Social Work program, or equivalent graduate degree in the helping professions (e.g., counseling or clinical psychology, marriage and family therapy, child and youth care)
Preferred Licensure or other certifications: CCM (Case Management Certification) or ACM (American Case Management Certification) preferred.. Knowledge of adoption and surrogacy policies and regulations. Expedites the discharge plan through excellent networking and team skills within the patient's expected length of stay per working DRG. Provides patient advocacy and ensures patient's Freedom of Choice and Medicare's Important Message.. Receives referrals for appropriate placement (NH, SNF, Assisted Living, LTAC, Acute Rehab etc., from Case Manager or Care Coordinator).. Encouraged to participated on hospital committees as related to post acute care services.
