INPATIENT SOCIAL WORKER LMSW or LCSW-C (PRN).. Position Summary: The Inpatient Social Worker at Sinai Hospital, in collaboration with the clinical team and medical provider, provides patient and family advocacy, discharge planning coordination, and psychosocial intervention for the high risk inpatient, to facilitate comprehensive and integrated care in the hospital and continuity of care in the community, post discharge from the hospital. Performance Improvement Stays abreast of changing care management and social work clinical trends, regulatory matters and third party payer requirements related to discharge planning and precertification of aftercare benefits. Requires a minimum of 2 years of inpatient hospital social work and discharge experience.. As a Y Family Services Manager , you will create and implement a Head Start recruitment plan for program participants to ensure 100% enrollment as well as establish and maintain a waiting list. The Clinical Social Worker provides comprehensive social work services for the patient population served and care management services to those patients in clinical programs..
