Identifies unsigned level of care orders and communicates with the utilization management nurse to obtain orders from providers.. Reviews current DRG/LOS identified within the electronic medical record system to assess discharge planning needs with providers and identifies the designated family member point of contact.. Performs discharge planning coordination and referral by making appropriate referrals to social services, ancillary departments, outpatient case management, durable medical equipment, post-acute placement, and other outside agencies according to Standard Operating Procedures.. Documents in the electronic medical record assessments, plans, interventions, barriers, and reassessments to facilitate discharges and transitions, manages anticipated discharge dates, and ensures all pertinent information is transferred to post-acute agencies..
