Coordinates clinical care (medical necessity, appropriateness of care and resource utilization for admission, continued stay, discharge and post- acute care) compared to evidence-based practice, internal and external requirements.. May delegate the implementation of the transition plan to LVN/LPN or Assistant staff.. Operates within the LVN/LPN scope of practice as defined by state licensing regulations.

Performs discharge planning coordination and referral by making appropriate referrals to social services, ancillary departments, outpatient case management, durable medical equipment, post-acute placement, and other outside agencies according to Standard Operating Procedures.. Documents in the electronic medical record assessments, plans, interventions, barriers, and reassessments to facilitate discharges and transitions, manages anticipated discharge dates, and ensures all pertinent information is transferred to post-acute agencies.. Represents the integrated care management department on various teams and performance outcomes committees and projects.

The Foster Care Case Manager acts as an advocate for children and families by working in conjunction with the state and courts to place abused, neglected or abandoned children into temporary foster homes to prepare them to return home or move to a permanent placement; provides professional contracted case management services to children, birth parents, and foster parents; formulates and assesses case plans and appears in court when necessary..
