The Crisis Case Manager II is a key member of the 24/7 Mobile Crisis Response Team, providing support to individuals of all ages in mental health crises.. Provide non-clinical care coordination services to individuals with serious mental illness (SMI) and substance use disorders (SUD).. May encounter risk of working with persons with SMI (serious mental illness) that may include, but not limited to persons in mental health crisis, persons that may be intoxicated or impaired by drugs or alcohol, persons who may be DTO (danger to others), DTS (Danger to Self), or GD (Gravely Disabled).. Solid understanding of working within a harm reduction framework to promote safety and well-being for individuals in crisis, along with knowledge of the cultural and socio-economic factors that influence service delivery for those with mental health and co-occurring disorders.. Critical thinking and problem solving with the Mobile Crisis Response Team to determine the best care plan for the person served.
Person in this position will be responsible for providing case management services including assessment of service needs, care, service planning, linkage and advocacy for persons served..
