Requires MA/MS in social work counseling or a related behavioral health field or a degree in nursing, and minimum of 3 years of clinical experience in social work counseling with broad range of experience with complex psychiatric and substance abuse or substance abuse disorder treatment; or any combination of education and experience which would provide an equivalent background.. Current active unrestricted license such as RN LCSW LMHC LICSW LPC (as allowed by applicable state laws) LMFT (as allowed by applicable state laws) or Clinical Psychologist to practice as a health professional within the scope of licensure in applicable states or territory of the United States required.. Previous experience in case management and telephonic and/or in person coaching with members with a broad range of complex psychiatric/substance abuse and/or medical disorders.

The facility-based Assistant Probation Officer position works within the Dakota County Juvenile Services Center's 24 hour secured juvenile detention facility and is responsible for providing direct supervision to clients for the purpose of ensuring a safe environment, meeting case-plan goals, preparing progress notes and incident reports while working as part of a treatment team.. Associate's degree or equivalent of two years post-secondary education in Corrections, Counseling, Criminal Justice, Education, Psychology, Sociology, Social Work, or related Human Services field AND one year experience in Corrections, Human Services Treatment Programs, or related experience.. WORK ENVIRONMENT: Work is performed within the juvenile detention center.. Convert any part-time experience to years (i.e. 20 hours week for 2 years equals 1 year full-time).

The role also includes community outreach to the Native American community and providing social work services across the continuum of care, including their return to the community.. Develop and revise treatment and complex discharge plans and communicate these with patients, caretakers, tribal entities, the treatment team, and direct care staff. Provide psychoeducation to patients, caretakers, the treatment team, and direct care staff regarding diagnosis, symptom management, recommendations, discharge plans (including leading discharge meetings), safety plans, patient rights, financial impact issues, Indigenous Health Disparities and other pertinent patient care matters across the continuum of care, including in the community.
