Remote Telehealth RN Virtual Care & Assessments

Fuze Health is seeking a Registered Nurse to support the telehealth care team within FuzeRx Central Operations. This remote role focuses on member outreach, ensuring adherence to Stars measures and ED discharge follow-ups, while delivering compassionate virtual care. The RN will conduct virtual assessments, document medical histories, educate patients, and collaborate with providers to develop patient‐centered care plans. A BSN and active RN license are required.
