Home Health Registered Nurse

Professional Case Management is seeking a Registered Nurse to provide in-home weekend care for clients with complex medical needs, including former energy workers and their families. In this full-time role, you will complete assessments, develop and manage care plans, administer medications and treatments, and closely monitor patient status. You will educate patients and families, collaborate with an interdisciplinary team, and document care using EMR. Provide direct in-home nursing care to clients with complex medical needs Assess, plan, implement, and evaluate individualized care plans Administer medications, treatments, and therapies per physician orders Monitor patient conditions and document clinical findings accurately Educate patients and families on disease management and home care Collaborate with interdisciplinary healthcare team members Ensure compliance with clinical protocols and safety standards Use EMR systems to document and communicate care Support weekend care coverage and respond to urgent needs Participate in ongoing training and professional development Patient assessment Care planning IV therapy Wound care Medication administration Electronic Medical Records (EMR) Care coordination Infection control Patient and family education Clinical documentation Medical Insurance Dental Insurance Vision Insurance Life Insurance 401k Active Registered Nurse (RN) license in state of practice BLS certification Preferred: ACLS or other advanced life support certification Home health or in-home care experience preferred
