Home Health Registered Nurse

Piedmont Healthcare is seeking a Registered Nurse for a 7-on/7-off home health schedule serving patients in Augusta, GA. In this role, you will provide comprehensive in-home nursing care, including assessments, care planning, wound care, medication management, and education for patients and families. You’ll collaborate with an interdisciplinary team to coordinate services and ensure safe transitions of care. Provide in-home nursing care, including assessments, treatments, and evaluations for home health patients Develop, implement, and update individualized care plans in collaboration with the healthcare team Administer medications, perform wound care, and manage IV therapies per physician orders Document all care accurately and timely in the electronic health record and required home health formats Educate patients and families on disease management, medications, and home care techniques Coordinate services with physicians, therapists, and other care team members to ensure continuity of care Monitor patient progress, recognize changes in condition, and escalate concerns promptly Adhere to infection control, safety, and regulatory standards in the home setting Participate in ongoing training and professional development offered by Piedmont Healthcare Support Piedmont’s patient-centered, respectful, and inclusive culture in all interactions Patient assessment Care planning Wound care Medication administration IV therapy OASIS documentation Electronic health records (EHR) Care coordination Patient and family education Infection control Medical Insurance Dental Insurance Vision Insurance Life Insurance 401k Active GA RN license BLS certification Home health experience preferred Valid driver’s license and reliable transportation OASIS documentation experience preferred
