Key Responsibilities: Conduct patient assessments, diagnose medical conditions, and develop appropriate care plans. Maintain accurate and up-to-date patient records.

Qualifications: ANCC Certification Valid NY Medicaid Provider status Previous experience in home health preferred Strong communication and interpersonal skills
