Job Description:
This per diem opportunity is designed for a skilled and compassionate Nurse Practitioner (NP) with a passion for providing home health assessments. You'll conduct home visits across Hudson County, NJ including Jersey City, Bayonne, and Union City performing detailed risk assessments for Medicare Advantage plan members. Each visit generally lasts about 45 minutes and may involve annual health checks or post-discharge assessments, offering a valuable chance to make a meaningful difference for the area's aging population.

Key Responsibilities:
- Complete annual home health risk assessments for Medicare Advantage members
- Carefully review and document current and prior medical history, medical conditions, and medication usage
- Provide insightful education for members regarding their health conditions and medications
- Detect gaps in care and recommend practical solutions as needed
- No requirement to prescribe medications or develop treatment plans
Qualifications:
- Active Nurse Practitioner license in NJBoard Certification via AANP or ANCCMSN degree from an accredited nursing program
- Current BLS/CPR certification
- At least 6 months of Nurse Practitioner experience preferred, especially in home health settings
