Key Responsibilities: Conduct annual home health risk assessments for Medicare Advantage members Evaluate current and prior medical histories, conditions, and medication use Deliver patient education about health conditions and medications Identify gaps in care, offer recommendations, and facilitate continuity of care Visits vary and may include annual assessments or post-discharge visits No treatment plan development or prescription of medications required

Qualifications: Active Nurse Practitioner License in Illinois Board Certification via AANP or ANCC required Current BLS (CPR) certification Master of Science in Nursing (MSN) from an accredited program At least 6 months of current NP experience preferred As part of this position, youll receive $80-$90 for each evaluation completed, in addition to a mileage stipend and compensation for administrative duties.
