Key Responsibilities:
Complete training for total access to Social Care Network (SCN) platforms
Schedule and support completion of non-clinical assessments while ensuring timely follow-up
Coordinate referrals across internal teams and external community-based organizations (CBOs)
Required Qualifications:
3-5 years of customer service or member engagement experience
Knowledge of Medicaid and/or Health-Related Social Needs (HRSN) programs
Proficiency in CRM or care management platforms
Strong communication skills
Problem-solving and active listening skills
