Home Health Care Coordinator

Key responsibilities: Manage home health and IV therapy referrals, assisting with intake and documentation as needed Promote positive relations with referral sources and ensure accuracy of demographic data and required documentation Coordinate with local vendors for services, negotiate rates, and maintain communication with hospitals and insurance carriers for safe discharges

Required qualifications: Some college education and/or a medical background; LPN preferred Experience in Work Comp Insurance, referrals, and authorizations is preferred Proficiency in MS Office applications, particularly Word, Excel, and PowerPoint Knowledge of medical terminology and injured workers' rights Ability to work collaboratively with co-workers and external customers with minimal supervision
