Licensed Practical Nurse – Long-Term Care

Signature HealthCARE is seeking a Licensed Practical Nurse to provide compassionate, resident-centered care in our long-term and post-acute facility. The LPN will deliver direct nursing care, administer medications and treatments, monitor resident conditions, and accurately document in the medical record. You'll collaborate with RNs, physicians, therapists, and support staff to carry out care plans that promote dignity, safety, and quality of life. Provide direct nursing care to residents in long-term and post-acute settings Administer medications and treatments per provider orders and facility policy Monitor, assess, and document resident conditions and responses to care Assist with care plan development and implementation with the interdisciplinary team Support ADLs, safety, and comfort while promoting resident dignity and rights Communicate effectively with residents, families, and healthcare team members Maintain accurate, timely electronic and/or paper medical records Follow infection control, safety, and regulatory standards Participate in quality improvement and facility initiatives Provide guidance to CNAs and support staff as delegated by supervising RN Medication administration Wound care Vital signs monitoring Care planning support Electronic medical record xevrcyc (EMR) documentation Infection control procedures IV therapy (per state scope and facility policy) Patient assessment and observation Geriatric nursing care Patient and family communication Medical Insurance Dental Insurance Vision Insurance Life Insurance 401k Current, active LPN license in state of practice BLS/CPR certification Experience in long-term care or skilled nursing preferred Knowledge of state and federal long-term care regulations Competence in electronic medical records documentation
