Remote NP/PA for In-Home Senior Care & Care Coordination

Alignment Health is seeking a Field Remote Nurse Practitioner or Physician Assistant to join the Care Anywhere (CAW) program in California. You will perform in-home assessments, medication reviews, and health screenings for Alignment members, while coordinating care with PCPs, specialists, and the care team. This hybrid role includes in-home visits two days a week and remote work three days, with a focus on preventing hospitalizations and advancing geriatric care.
