What You'll Do: Provide skilled nursing care and health counseling to patients in their homes. Support treatment goals, rehabilitation potential, and the reduction of pain and other symptoms. Complete initial visits to assess patients' physical and emotional status, needs, and available resources. Gather and analyze information through observation, interviews, and record review. Develop nursing plans of care based on assessment findings and the physician's plan of treatment. Provide patient education using the established care plan and standards of care. Deliver professional nursing services in accordance with agency standards and policies.

What You Need Required: Registered Nurse (RN) license. Ability to provide professional nursing care in patients' homes. Ability to assess patient needs, develop nursing care plans, and provide patient education.
